Have you received an indefinite disqualification and been ordered to attend an Assessment Centre due to repeat offending that involves alcohol or drugs?
Have you been disqualified recently because you drove whilst disqualified, suspended or contrary to the terms of a Work Licence?
Have you been convicted of any two of the following offences, committed on separate occasions, WITHIN FIVE YEARS of each other:

driving under the influence of alcohol or drugs?
reckless or dangerous driving?
careless driving causing injury or death?
failing to stop after an accident?
applying for, or obtaining, a licence while disqualified from doing so?

In addition to the above does any of the following apply to you:

You conduct driving tests or act as a driving instructor.
You drive in a passenger service (except where demerit points were the only reason for the licence suspension and where no more than 20 of these points were accumulated while driving in a passenger service).
You drive in a transport service, if you were disqualified for excess alcohol while operating a transport service vehicle.
